Class Information – Which class should I enroll in?
Mommy and Me Preschool Ballet
Introductory ballet class – 45 minutes
Ages 3 and 4 (must be 3 by September 1st)
Recommended for preschool age beginning dancers
Each dancer brings an adult with them to class. The adult stays with their dancer and assists as dancers learn how to execute beginning ballet positions and steps
Ballet 1
Introductory ballet class – 45 minutes
Age 4-6 (Pre/K)
Must be 5
Preschool age dancers with ballet experience can also enroll
Ballet 2
Ballet class – 60 minutes
Ages 5-7 (K/1st)
1 year ballet experience
Ballet 3
Ballet class – 60 minutes
Ages 7-9 (grades 2-3)
2+ years ballet experience
Ballet 4
Ballet class – 75 minutes
Ages 9-11 (grades 4-5)
3+ years ballet experience
Ballet 5
Ballet class – 75 minutes
Ages 11-14 (grades 6+)
3+ years ballet experience
Jazz/Contemporary Technique
Jazz class – 60 minutes
Ages 11-14 (grades 6+)

Studio Policies
Tuition and Fees

- Tuition is a monthly fee and not per class bases. Payment is the same regardless of the number of classes your child attends. In the case that a class must be cancelled, parents will be notified and a make-up class will be scheduled.

A registration fee of $15 per family will be due at the time of registration and holds your child’s spot in the class.

Family Discount: A discount of 10% will be applied for additional students from the same family. The discount will be applied to the lowest of the tuition amounts.

Costume fees will vary based on the number of costumes they will wear in the recital. Costume fees are due December 1. Students can keep costumes after the spring recital.
Attendance/Withdrawal Policy
Students are expected to attend class as often as possible. However, it is very important to Summit to keep all dancers healthy, so if your child has any symptoms of illness please do not send them to class. We do not want to call parents to pick up sick students.
Our Spring Recital will take place at the end of the season. Students will be required to attend the Recital as well as Tech/Dress Rehearsals leading up to the Recital. Additional Recital and rehearsal details will be sent out during the season.
Ballet Attire
Students must wear appropriate ballet attire. This includes the following:
– Plain black leotard (can be tank/straps, short sleeved, or long sleeved)
– Pink footed ballet tights (these can be convertible or solid footed tights, but must be inside ballet shoes for class).
– Pink ballet shoes – these are flat ballet slippers in ballet pink (not pointe shoes)
– Hair in a bun – pulled back and away from the face.
Jazz Attire
– Hair pulled back and away from face
– Leggings, shorts, or other exercise clothing
– Form fitting shirt or tank top
– Bare feet, jazz shoes, or bear claw footwear are all acceptable
Waivers
Student/Staff Waiver & Release of Liability
I understand that Summit Ballet and Dance does not give credit and/or refunds for class(es) missed due to
holiday, vacation, illness, weather, etc.
I further understand that there are risks of physical or property damages, losses, illness or injury inherent in any
exercise or dance program. I am participating with the expressed agreement and understanding that I am hereby
waiving and releasing Summit Ballet and Dance from and against any and all claims, costs, liabilities, expenses
or judgements including attorney’s fees and court costs arising out of my/my child’s participation in this program.
I hereby agree to indemnify and hold harmless [XYZ Studio] authorities from and against any and all such claims.
Consent and Waiver Regarding the Use of Student/Staff Photographs and Videos
I hereby allow photos and videos of myself/my student to be published via print, video, website or social media
which are affiliated with Summit Ballet and Dance.
I understand that publication may be accomplished electronically via the internet/world wide web and that after
publication the school will be unable to prevent persons from gaining access to the internet/world wide web,
copying my photographs and video therefrom and subsequently using, altering or republishing it without my
consent.
I waive any claim for damages against Summit Ballet and Dance from the unconsented-to-use, alteration,
republication of my photographs and videos by third parties accessing the internet/world wide web or obtaining
copies of the print or video material.
Tuition and Fees
Tuition is a monthly fee and not per/class based. Payment is the same regardless of the number of classes your
child attends. In the case that a class must be cancelled, parents will be notified and a make-up class will be
scheduled. Tuition is due the first of each month. There will be a $5 late fee if payment is not made by the last
day of the month. A registration fee of $15 per family will be due at the time of registration and will hold your
child’s spot in the class. Costume fees will be dependent on the number of costumes each dancer wears in the
recital. Costume fees are due December 1. A 10% family discount will be automatically applied for additional
students from the same family. The discount will be auto-applied to the lowest of the tuition amounts.I accept
that by registering for classes, I agree to pay in full all tuition and fees for all classes I register for.
Acknowledgement
I have reviewed Summit Ballet and Dance policies and will take the responsibility to carefully read and follow the
rules and policies therein. I understand that Summit Ballet and Dance does not give credit and/or refunds for
class(es) missed due to holiday, vacation, illness, weather, etc. I further understand that there are specific risks
of physical or property damages, losses, or injury that may result from my or my child’s participation with Summit
Ballet and Dance, and I voluntarily assume the risks associated with such participation.
